New Patient Registration
To register with the practice you must be permanently resident in our catchment area (see below).
You will be asked to show our receptionist two pieces of evidence containing your name and address (e.g. utility bills, medical card). If these are provided she will then make a registration appointment for you to see our HCA. Once your registration is complete you will be able to receive services from us and consult with our doctors.
Please attend on time for your appointment. If you arrive late, your appointment will have been missed and you will be required to arrange a further time. Failing to keep a registration appointment will result in no further appointments being given.

Change of Personal Details
Please notify the surgery as soon as possible if any of your personal details change. This may be your address, telephone number or next of kin contact details. The receptionist will take your new details and alter your medical records accordingly.
